Jacob Forman is a screenwriter and producer. His credits include the screenplay for *All the Boys Love Mandy Lane* (2006) and *Uglies* (2024), and episodes of the series *Lawmen: Bass Reeves* (2023) and *Dutton Ranch* (2026). He also produced *The Last Survivors* (2014).

Forman has worked in writers’ rooms and as a script consultant. He taught screenwriting at the American Film Institute in Los Angeles and is a member of the Writers Guild of America West. He holds American and Brazilian citizenship.
