Biography
Andrew Form is an American film and television producer. Born on 3 February 1969 in New York City, he co-founded the production company Platinum Dunes with Michael Bay and Brad Fuller in 2001.
He has produced numerous horror and action films, including The Purge (2013) and its sequels, Teenage Mutant Ninja Turtles (2014), A Quiet Place (2018) and its follow-ups A Quiet Place Part II (2021) and A Quiet Place: Day One (2024), as well as IF (2024). Form has also worked in television, serving as executive producer on series such as Jack Ryan (2018–2023) and Black Sails (2014–2017). In 2018, he and Brad Fuller launched Fully Formed Entertainment.
